Is Tailored Transitions considered an inpatient or outpatient program? Well, actually it could be considered the only model to fall within both categories. Individuals do not check-in to a traditional inpatient rehab facility. In a sense, your home becomes the inpatient facility. Participants are not restricted (or isolated) at home, but are encouraged at some point in the process to live their lives accordingly (minus a few key things of course). This model blends inpatient and outpatient approaches to deliver a unique level of care unlike traditional programs. We offer a complete live-in approach to clients internationally. Our recovery coaches and sober companions travel to stay with clients, and accompany them wherever they may need to go allowing for flexibility and the continuation of life’s responsibilities (i.e. work).
We also offer Intensive Outpatient Services (IOP) which involve individual sessions and family sessions for those that have completed our live-in programs, OR for those that are not in need of such intensive services. Our Intensive Outpatient Services are offered in the Silicon Valley and surrounding areas as our main office resides in the Bay Area, CA. For some, an inpatient facility may be appropriate as some are in need of medication-detoxification assistance, or a temporary reprieve from the home/work environment is warranted. Tailored Transitions supports traditional rehabilitative programs, and sometimes coordinates with select rehab/detox facilities. Tailored Transitions, LLC can be used as a program designed to facilitate change before (pre-care) the admittance into a rehab. Furthermore, the program is also an ideal aftercare program to assist with an individual’s transition back into their life. For most individuals, Tailored Transitions’ holistic approach can be used as a standalone treatment to address the needs of our clients. HEALTH AND FITNESS
Recovering from addiction and achieving long-term relapse prevention involves a gradual overall change in lifestyle. Our program includes educating clients on making thoughtful decisions regarding nutrition and exercise. Staff is available to design resistance (weight training) and cardio vascular training programs. In some situations we may refer clients to local health clubs, and accompany individuals as they begin mild to moderate weekly exercise regiments. We believe in promoting health and fitness throughout the recovery process to achieve successful outcomes with overall wellness. Many individuals challenged with addiction issues have neglected their health as they eat processed foods, consume harmful substances, refrain from sleeping, and live a sedentary lifestyle. Our holistic approach involves not only refraining from polluting our bodies, but making lifestyle choices where we befriend ourselves and nurse our bodies back to an optimal state.
It should be noted that all recovery coaches and licensed clinicians employed with Tailored Transitions work within their scope of competency and practice. Our licensed mental health clinicians may address issues that are hindering the progress of treatment and overall well-being (i.e. depression, anxiety, trauma, etc.). Our life coaches work toward increasing intrinsic motivation and maintaining accountability so individuals can achieve optimal health. However, we do not make recommendations of specific dietary needs which may be outside of our scope. When warranted our staff will collaborate with the appropriate medical or nutritional practitioners through the formation of an interdisciplinary team. The linkage to, or collaboration with, third-party experts will be done only after consulting with a client and obtaining proper consent.
In some cases, a temporary reprieve from the home/work environment may be a valuable component of our support plan. Taking trips can be a tempting time for those challenged with addiction as they have a tendency to succumb to cravings. It is important to learn to have fun and relax on vacations while utilizing the tools of recovery. However, we caution our clients not to use vacations (and sometimes luxury resort like rehabs) as another form of escaping from life. In a sense, this can be another maladaptive coping mechanism. At some point the vacation must end and you must return to your home, work, and family. As you reintegrate into everyday life it will be crucial for you to have the support in place to cope with the inevitable stressors and temptations awaiting you at home.
